Air Defense Complex, shot down a Ukrainian liner near Tehran, was previously moved and not reconfigured, Iranian Civil Aviation Organization Supplemented Report, incident investigation.
As follows from the report, at night, Iran launched a missile strike on an American military base in Iraq, then “the level of readiness of the country's air defense was changed”. In preparation for the possible repulsion of an air attack, one of the air defense systems was moved, but the new setting was not made on it.
One of Tehran's air defense systems was displaced. This led to a change in its geographical position and direction.. (…)after moving the reconfiguration was not performed

The report indicates, that due to the movement of the air defense system and incorrect settings, the complex system mistakenly recognized the Ukrainian liner as an object, approaching Tehran, although in fact the plane was moving away from the Iranian capital.
Second mistake, resulting in a plane crash, it was, that there was no data exchange between the air defense complex and the focal point.

Recall, that ukrainian passenger plane, flying out of tehran, crashed in the morning 8 January 2020 of the year. In the crash died 176 human. 11 January Iranian military said, that the liner was shot down by accident, and the reason for this was “human factor”.
